  • REGISTRATION OF FOREIGN CERTIFICATES IN POLAND

  • 1. REGISTRATION OF FOREIGN BIRTH, MARRIAGE AND DEATH CERTIFICATES IN POLAND

     

    In order to apply for Polish passport it is necessary to possess Polish birth and/or marriage certificate. People who were born or got married abroad can register their foreign birth and marriage certificates in Polish Registry Office personally, by means of an authorized third person or through the Consular Section. As a result they will obtain Polish certificates.

     

    2. REGISTRATION OF A BIRTH CERTIFICATE OF AN ADULT

     

    Following documents should be submitted personally at the Consular Section:

    - the original foreign birth certificate with apostille (SEE.APOSTILLE)

    - a translation of the birth certificate – (SEE.TRANSLATION);
    - application to the Head of the Registry Office of your choice (SEE.FORMS TO PRINT. ATTACHMENT 8);

    - valid Polish or foreign passport of the applicant.

     

    REGISTRATION OF A BIRTH CERTIFICATE OF A MINOR

    Birth certificates of minors, born abroad, are registered on the basis of a written application, submitted by one of the parents to the Head of the Registry Office of your choice, along with the original birth certificate and its certified translation.
     

    REGISTRATION OF A BIRTH CERTIFICATE OF A MINOR

     

    - the original foreign birth certificate with apostille (SEE.APOSTILLE)

    - a translation of the birth certificate – (SEE.TRANSLATION);
    - application to the Head of the Registry Office of your choice  (SEE.FORMS TO PRINT. ATTACHMENT 14);

    - valid Polish or foreign passport of at least one parent

    - valid foreign passport of the minor.

     

    Parents are strongly advised to present their own Polish birth and/or marriage certificates.

     

    REGISTRATION OF MARRIAGE CERTIFICATE

    Marriage certificates are registered in the Polish Registry Office of your choice.

    Following documents should be submitted personally by both spouses at the Consular Section:

    - the original foreign marriage certificate with apostille (SEE.APOSTILLE)

    - a translation of the marriage certificate –(SEE.TRANSLATION);
    - application to the Head of the Registry Office of your choice (SEE.FORMS TO PRINT. ATTACHMENT 9);

    - valid Polish or foreign passports of both spouses.

     

    A WRITTEN STATEMENT CONCERNING A CHOICE OF A LAST NAME

    Such a statement may be required if the original marriage certificate does not contain information about a change of a surname as a consequence of a marriage celebration.

     

    REGISTRATION OF A DEATH CERTIFICATE


    An application for registering a foreign death certificate can be submitted by a person who can prove a ‘legal or actual interest’ in registering death certificate

     

    Following documents should be submitted personally at the Consular Section:

    - the original foreign birth certificate with apostille (SEE.APOSTILLE)

    - a translation of the birth certificate – (SEE.TRANSLATION);
    - application to the Head of the Registry Office of your choice (SEE.FORMS TO PRINT. ATTACHMENT 8);

    - valid Polish or foreign passport of the applicant.
